The plugin page says:
FTP Address: The path and file to for the zip file i.e. C:\output.zip
 
What does that mean?
There is a typo in "to for".
Surely the ftp address is just a URL?
Can it just be ftp.my.server.com?
Or should it be ftp://ftp.my.server.com?
Do we need the port?
 
Remote Folder: The remote folder destination '/' for root.
Can we use date patterns for the remote folder?
i.e. so that it daily goes to a different sub-dir?

I know you asked this forever ago (seems I was not surbscribed to this issue tracker)

address-> yup just put the name or ip there ftp:// is not required
port -> its going to assume port 2
remote folder -> you would have to use a build parameter in the path, something like /myproject/%date%/

Though, turns out that teamcity does not exposes that without another plugin.
